Periods

Heavy menstrual bleeding

Excessive or prolonged menstrual bleeding that affects daily life. Almost always treatable once the cause is identified.

Understanding

Heavy menstrual bleeding.

Heavy periods are common, but they are not something you simply have to live with. Heavy bleeding may be caused by fibroids, polyps, hormone imbalance, conditions of the womb lining, or sometimes nothing structural at all.

A focused review with Mr Orabi usually identifies the cause within one appointment, and the treatment options range from medication and the Mirena coil through to outpatient hysteroscopy and surgery.

Common symptoms

What to look out for.

  • Periods lasting longer than seven days

  • Soaking through pads or tampons every one to two hours

  • Passing large clots

  • Flooding through to clothing or bedding

  • Tiredness or breathlessness from anaemia

Diagnosis

How it's investigated.

Approach

A structured workup, often within one appointment, to identify the cause and shape a clear treatment plan.

  • Detailed cycle history

  • Pelvic ultrasound (abdominal and transvaginal)

  • Endometrial biopsy when indicated

  • Hysteroscopy to look directly inside the womb

  • Blood tests for anaemia, thyroid and coagulation

Treatment

Tailored to your priorities.

Treatment is built around your symptoms, preferences and any other conditions. Options often combine medication, hormonal treatments and where appropriate, surgery.

  • Tranexamic acid and anti-inflammatories during periods

  • Hormonal options: combined pill, mini-pill, depot injection, implant

  • Mirena (intrauterine system), highly effective for reducing flow

  • Endometrial ablation in selected cases

  • Surgical treatment including myomectomy or hysterectomy where appropriate
