Amenorrhoea (absent periods)

Absent periods, either primary (periods never started) or secondary (periods have stopped). There is almost always an identifiable cause.

Understanding

Amenorrhoea (absent periods).

Absent periods are common and can happen for many reasons: polycystic ovary syndrome, thyroid imbalance, low body weight, stress, hyperprolactinaemia, or in some cases premature ovarian insufficiency.

A structured workup almost always finds the answer. The plan then focuses on whichever combination of fertility, hormonal health and long-term bone protection is most important to you.

What to look out for.

  • No periods by the age of 15 (primary amenorrhoea)

  • Periods that have stopped for three months or longer (secondary amenorrhoea)

  • Other features: weight change, acne, hair growth or loss, headaches, milk discharge

  • Detailed menstrual and lifestyle history

  • Hormonal blood tests (FSH, LH, oestradiol, prolactin, TSH, testosterone, AMH)

  • Pelvic ultrasound

  • MRI of the pituitary gland in selected cases

Treatment is built around your symptoms, preferences and any other conditions. Options often combine medication, hormonal treatments and where appropriate, surgery.

  • Treat the underlying cause (weight, stress, exercise, thyroid imbalance)

  • Hormonal regulation (combined pill or cyclical progestogen) where appropriate

  • Treatment of polycystic ovary syndrome (PCOS)

  • Fertility support where pregnancy is the goal

  • Hormone replacement when premature ovarian insufficiency is identified
